Are Mail-Order Aligners Safe? What to Know Before You Order Teeth Straightening Online
Mail-order aligners aren't considered safe without in-person supervision from a dentist or orthodontist. Clear aligners are prescription medical devices, and in September 2026 the FDA updated its consumer guidance to say it has not authorized aligners sold directly to consumers without a prescription and professional supervision.
I understand the appeal. Adults in Andover and Wichita often ask me about mail-order aligners: a lower price, no appointments, a box on the doorstep. But moving teeth means moving living bone and gum tissue, and what happens beneath the surface matters as much as what you see in the mirror.
Here's what the FDA now says, what can go wrong, and how to choose a safer path.
Are at-home clear aligners safe?
The FDA's position is direct: clear aligners are prescription devices, safe to use only under the supervision of a licensed dentist or orthodontist. The agency says an in-person exam can lower the risk of an uneven bite, unintended tooth movement, pain, and tooth loss.
The FDA has also received reports of new tooth or jaw pain, bite problems, worsening misalignment, and new gaps from non-authorized direct-to-consumer aligners.
The American Association of Orthodontists (AAO) strongly discourages mail-order treatment, noting that poorly fitted appliances can cause permanent damage to teeth and gums.
What can go wrong without in-person supervision?
Most of what I monitor can't be seen in a photo upload:
No X-rays or hands-on diagnosis. X-rays reveal tooth roots, bone, missing teeth, and jaw position. The AAO calls them essential to an accurate diagnosis, and most mail-order programs skip them.
No attachments. Small tooth colored bumps called attachments are bonded to certain teeth so aligners can grip and guide them. Without them, some movements become unpredictable.
No progress exams. Regular visits help me catch problems early: root shortening (root resorption), gum disease, white spots under the aligners, and jaw joint discomfort.
No one to change course. When a tooth isn't tracking, I can adjust the plan. A box of trays shipped all at once can't.
What's the difference between doctor directed and DIY aligners?
Doctor-directed treatment means an orthodontist examines you in person and sees you throughout. With DIY aligners, you take your own impressions and any review happens remotely.
| Orthodontist-supervised aligners | Mail-order aligners | |
|---|---|---|
| In-person exam | Yes | Usually not included |
| X-rays before treatment | Yes | Usually not taken |
| Attachments on teeth | Placed in office | Not possible without an office visit |
| Progress checks | In-person visits | Remote check-ins, if any |
| Who oversees care | An orthodontist you've met | Often a provider you never meet |
Some companies call remote review "doctor-directed." The FDA notes that virtual visits can be useful, but periodic in-person visits help reduce risk.
What happened to SmileDirectClub?
SmileDirectClub, once the best known mail order aligner company, filed for Chapter 11 bankruptcy in September 2023 and shut down that December. Patients mid-treatment were left without the company coordinating their care.
The lesson isn't simply that one company failed. It's that orthodontic treatment is an ongoing healthcare relationship, not a product that arrives in the mail. New direct-to-consumer brands keep appearing, so always ask who is supervising your care.
Are mail-order aligners really cheaper?
The advertised price is often lower, and cost is a real concern worth discussing openly. Part of the difference is what's left out: the exam, X-rays, attachments, and progress visits.
The AAO warns that unsupervised treatment can lead to irreversible and expensive damage, which can make a lower price cost more over time. Many dental plans with orthodontic coverage also cover clear aligners and braces equally. For local numbers, see what orthodontic treatment actually costs in Andover.
Why do I need an orthodontist for Invisalign instead of ordering aligners online?
FDA-authorized aligners, including well-known brands like Invisalign, are prescribed through a dentist or orthodontist. The trays are only one part of treatment; the diagnosis, plan, attachments, and check-ins are what make results predictable.
At Sander Orthodontic Arts, we offer Angel Aligners, a clear aligner system I prescribe and supervise from your first scan to your final retainer.
I've already started mail-order aligners. What should I do?
If you notice new pain, a bite that feels off, or teeth shifting the wrong way, the FDA recommends contacting a dentist or orthodontist for an evaluation. I'm always glad to take a careful, no-pressure look.
What does orthodontist-supervised aligner treatment look like in Andover?
For patients across Andover and the greater Wichita area, treatment begins with an in-person consultation: an exam, a digital scan, X-rays, and an unhurried conversation about your goals. If aligners are right for you, we place attachments, fit your first trays, and see you regularly to confirm everything is on track.
If aligners aren't the best fit, I'll tell you honestly. Orthodontics is both a science and an art, and the artistry depends on seeing the whole picture.

Frequently asked questions
Are mail-order aligners safe? According to the FDA, clear aligners are prescription devices that are only safe under the supervision of a licensed dentist or orthodontist. The FDA has not authorized aligners sold directly to consumers without a prescription and supervision.
What are the risks of DIY clear aligners? Without in-person exams and X-rays, problems below the gumline can go undetected. The FDA has received reports of new tooth and jaw pain, bite problems, worsening misalignment, and new gaps between teeth from non-authorized direct-to-consumer aligners.
Is it safe to straighten teeth without an orthodontist? The FDA says in-person exams and periodic in-person visits help reduce the risks of unintended tooth movement, bite problems, pain, and tooth loss.
How can I tell if clear aligners are FDA-authorized? Ask your dentist or orthodontist which product they use, or search the FDA's 510(k) database using product code NXC. "FDA-registered" only means a company has listed its facility and products, which is not the same as FDA-authorized.
What happened to SmileDirectClub? SmileDirectClub filed for Chapter 11 bankruptcy in September 2023 and shut down in December 2023. Many patients were left mid-treatment without the company that had been coordinating their care.
Is there a SmileDirectClub alternative in Wichita? Yes. Sander Orthodontic Arts in Andover offers doctor-directed clear aligners, prescribed and monitored in person by an orthodontist, for patients throughout the Wichita area.
